When several organisations or professionals are involved in a person’s support, information can easily become fragmented. Our role is to contribute to a coordinated approach in which the person’s needs, risks, strengths and goals remain visible throughout assessment, transition and ongoing support.

We therefore treat partnership working as an ongoing part of the placement rather than something that ends once funding and accommodation have been agreed.

What You Can Expect

A clear and responsive approach to placement partnerships.

Our role is not simply to accept a referral. We consider whether the proposed service can appropriately meet the person’s needs and how the placement can be supported over time.

Responsive Assessment

We review available referral information to understand the person’s circumstances, support requirements, identified risks, strengths, preferences and intended outcomes before determining whether we can offer an appropriate placement.

02 / COMMUNICATE

Clear Communication

We aim to maintain appropriate communication with commissioners and involved professionals so that material changes, emerging concerns, progress and barriers can be discussed rather than allowed to develop in isolation.

03 / PERSONALISE

Individual Support Planning

Support arrangements are developed around the person’s assessed needs and circumstances. This creates a clearer relationship between identified needs, planned support, risk management and the outcomes the placement is intended to achieve.

04 / REVIEW

Active Review

A person’s circumstances are not static. We recognise when support arrangements, risks or priorities require reconsideration and contribute to reviews with relevant professionals where changes may be necessary.

The placement agreement is the beginning of the partnership.

Effective partnership requires communication to continue as the person settles, develops and encounters new challenges. The information required will differ between placements, but communication should remain relevant, proportionate and focused on matters that affect the person’s support.

In these situations, effective partnership is particularly important because placement stability can depend on timely information, clear responsibilities and coordinated risk management.

We consider each referral individually and do not assume that a diagnostic label or previous history alone determines whether a placement is suitable.

An Active Partnership

Working together when circumstances change.

Not every placement develops exactly as anticipated. A person’s risks, presentation, aspirations or support needs may change, and new information may emerge after a placement has begun.

Effective partnership means recognising these changes early and considering them collectively rather than waiting until difficulties become unmanageable.



When review may be needed

  • There is a significant change in the person’s support needs or risks.
  • Existing arrangements are no longer achieving their intended outcomes.
  • New safeguarding or risk management concerns emerge.
  • The person has developed greater independence and support may be adjusted.
  • A transition to a different level or model of support is being considered.
  • Professionals identify barriers that require a coordinated response.
