From your initial enquiry through to moving into one of our supported living services, our experienced referrals team works closely with individuals, families, Local Authorities, NHS services and healthcare professionals to ensure every placement is carefully assessed, safely planned and centred around achieving positive long-term outcomes.

A straightforward and supportive referral pathway

Every referral is unique. Our application process has therefore been designed to remain flexible whilst ensuring every placement is clinically appropriate, safe and capable of supporting long-term independence and wellbeing.

Throughout each stage we maintain close communication with commissioners, clinicians, social workers, family members and the individual themselves, ensuring everyone remains informed from referral through to admission.

Support begins immediately following admission with regular reviews to monitor progress and outcomes.

Funding & Referrals

Paying for Your Support

Funding arrangements differ for every individual. Whether support is arranged through a Local Authority, NHS organisation, Personal Budget or Direct Payment, our experienced team will guide you through every stage whilst working closely with commissioners and healthcare professionals throughout the referral process.

Personal Budgets

Following a Care Act assessment, eligible individuals may receive a Personal Budget to meet assessed care and support needs whilst promoting greater independence, flexibility and personal choice.

Direct Payments

Some individuals choose to receive funding directly, enabling them to select providers and tailor support arrangements around their own goals, preferences and lifestyle.

Commissioned Placements

Many placements are commissioned directly by Local Authorities, NHS Trusts or Integrated Care Boards following assessment, approval and collaborative transition planning.

Timescales vary depending upon the availability of assessments, funding approvals, accommodation and individual circumstances. Our referrals team provides regular updates throughout the process.
