Time to move on
The length of time you receive support will depend on your individual needs, goals, progress and the type of service involved. We will explain what applies to your service and discuss any expected timescales with you.
Your support will be reviewed regularly. When you are ready for a change, we will work with you and the other people involved in your care to plan the next step safely and carefully.
Ready to live independently
Your Support Worker will help you develop the skills, confidence and knowledge you may need for greater independence. This may include managing your home, attending appointments, looking after your wellbeing, managing money or accessing your local community.
As you make progress, the amount or type of support you receive may change. Any changes will be discussed with you and carefully planned so that you continue to receive appropriate support.
Finding a new place to live
If your next step involves moving into different accommodation, we will help you understand the available options and what needs to happen before your move.
Housing options will depend on your individual circumstances, local availability, eligibility and funding arrangements. Where appropriate, we can help you contact housing services, local authorities or other organisations that may be able to provide advice.
