About the role

Moreways Healthcare supports adults with mental health needs, learning disabilities, autism, complex emotional needs and other conditions that may affect independence and participation in community life.

We also support individuals progressing through forensic and step-down pathways, including people moving from hospital, secure settings or other highly structured environments into supported living and increasingly independent community life.

The Activity Hubs Service Manager will lead the development and coordination of Moreways Healthcare’s structured programme of meaningful activity, community reintegration, skills development and progression opportunities.

This is not intended to be a traditional day-centre model in which people are simply brought together to occupy time. The purpose is to create practical pathways that help individuals develop confidence, routine, transferable skills, social connections and greater independence.

Activity may be delivered within Moreways Healthcare services, through community venues, through external partners or through individual opportunities such as volunteering, education and employment preparation.

Ensure activity connects directly with individual goals rather than existing solely to fill people’s time.

Key responsibilities

  • Lead the development of Moreways Healthcare’s programme of structured activities, skills development and community participation.
  • Ensure activity provision reflects individual support plans, strengths, interests and agreed outcomes.
  • Develop opportunities that promote independence rather than unnecessary reliance on provider-led activities.
  • Coordinate activity and progression opportunities across supported living, outreach and community services.
  • Develop partnerships with education providers, community organisations, voluntary groups, leisure services and employers where appropriate.
  • Support individuals to access volunteering, education, employment preparation and skills-development opportunities.
  • Develop practical workshops around independent living, digital inclusion, budgeting, cooking, travel and other relevant areas.
  • Promote physical activity, wellbeing and healthy routines through appropriate community opportunities.
  • Ensure activities are inclusive and adaptable for people with autism, learning disabilities, mental health needs and differing communication requirements.
  • Ensure risk-management arrangements are considered when activities take place outside supported living services.
  • Develop procedures for planning, recording and evaluating activities and community engagement.
  • Monitor participation and outcomes rather than simply recording attendance.
  • Provide leadership and direction to staff involved in delivering activity and community programmes.
  • Ensure staff receive appropriate induction, supervision, training and development.
  • Develop service standards and quality measures for activity and community-integration work.
  • Contribute to individual reviews and provide evidence of progress where activity forms part of a person’s agreed outcomes.
  • Maintain effective communication with Service Managers and frontline teams across the organisation.
  • Identify gaps in current provision and develop practical solutions.
  • Monitor budgets associated with activity provision and ensure resources are used effectively.
  • Contribute to service development proposals, commissioner discussions and new initiatives where appropriate.

From activity to progression

Meaningful activity should have a purpose. For one person, the starting point may simply be leaving the house regularly. For another, it may be travelling independently, enrolling on a course or preparing for employment.

The programme should therefore provide different stages of opportunity rather than assuming everyone requires the same intervention.

Community reintegration is central to the role

Moreways Healthcare’s longer-term objective is to help people build sustainable lives outside formal support systems wherever this is possible.

This is particularly important for individuals moving through forensic or hospital step-down pathways, where a long period within institutional or highly structured environments may have reduced opportunities to develop ordinary community roles and relationships.

Community reintegration may therefore involve learning to travel independently, using local facilities, managing appointments, joining groups, volunteering, rebuilding social confidence or preparing for education and employment.

The aim is not simply to increase activity. It is to increase capability, confidence, social inclusion and meaningful participation.

Supporting people with complex and forensic needs

Some individuals supported by Moreways Healthcare have significant mental health or forensic histories and may require structured risk-management arrangements while participating in community activities.

Depending on the individual, arrangements may include Section 37/41 conditional discharge, Section 117 aftercare, MAPPA involvement, forensic community-team oversight or other professionally agreed restrictions.

You will ensure activity planning takes appropriate account of these arrangements without using risk as a reason to exclude people unnecessarily.

Good planning should identify how participation can occur safely and progressively, including appropriate staffing, travel arrangements, contingency planning and information sharing where required.

Leading and developing the team

Leadership Set a clear direction and ensure staff understand that meaningful activity should lead towards individual outcomes.
Supervision Provide structured supervision, performance management and professional development.
Coaching Help staff develop confidence in engagement, motivational approaches and community-based support.
Consistency Develop systems that allow activity and progression work to continue consistently across different services.
Quality Review records, participation data, feedback and outcomes and address weak practice.
Innovation Identify new opportunities and test approaches that improve community participation and progression.

Performance and outcome measurement

The Service Manager will be expected to demonstrate what difference the programme is making.

Attendance alone is not sufficient evidence of success. Performance monitoring should consider whether individuals are developing skills, becoming more confident, increasing independent community use, building social connections or moving towards education, volunteering or employment.

You will develop appropriate reporting arrangements and provide senior managers with clear information about participation, outcomes, barriers, capacity and areas requiring development.

Where an activity or intervention is not producing meaningful benefit, you should be prepared to review or redesign it.

Partnership development

Much of the best community activity should take place outside Moreways Healthcare rather than within services created solely for people receiving support.

Building strong external partnerships will therefore be a significant part of the role.

What we are looking for

Service leadership Experience managing a social care, community, rehabilitation or related service.
Community development Ability to identify opportunities and build relationships outside the organisation.
Outcome focus Ability to distinguish between activity for its own sake and activity that creates meaningful progression.
Complex needs Understanding of mental health, learning disability, autism, forensic or other complex support needs.
Staff leadership Ability to supervise, coach, develop and hold staff accountable.
Performance management Confidence using data and qualitative evidence to monitor quality and outcomes.
Partnership working Ability to develop credible relationships with community organisations and professional partners.
Innovation Ability to identify gaps, develop practical ideas and take responsibility for implementation.

Experience and qualifications

Previous management experience within supported living, mental health, learning disability, autism, community rehabilitation, forensic services or another relevant environment would be advantageous.

Experience of developing community programmes, volunteering, employability, education, skills development or social-inclusion initiatives would be particularly relevant.

A relevant Health and Social Care, Management, Community Development or similar qualification would be beneficial, although substantial relevant experience will also be considered.

You should be confident using digital systems, preparing reports, monitoring performance and communicating professionally with internal and external stakeholders.
