Accessing Support

Finding the right support starts with understanding where you are now — and where you want to get to. You may already know that supported living is right for you, or you may simply know that something needs to change. You do not need to understand every service model before speaking to us. We can help you make sense of the options, understand what matters most and identify the next appropriate step.

Start Where You Are

You do not need to know the answer before you contact us

Adult social care can come with a lot of terminology: supported living, outreach, step-down, community support, forensic pathways, 1:1 support and many other descriptions. Those labels can be useful to professionals, but they should not become a barrier to somebody finding the right help.

What matters first is understanding the person: their strengths, preferences, current difficulties, safety, daily life, relationships, health, community, existing support and hopes for the future. The support model should follow from that understanding.

1
What is working well?Good support should build on strengths, routines and relationships that already help.
2
What is becoming difficult? We look at where additional structure, skills, reassurance or specialist support may be needed.
3
What matters to you?Home, independence, safety, work, relationships, community and personal goals can all shape the right plan.
4
What would progress look like?Progress may mean greater independence, more stability, fewer restrictions, stronger routines or simply feeling safer and more in control.

Not Sure Where To Start?

Tell us the situation — not the service name

You do not need to diagnose the support model yourself. Different starting points can lead to different Moreways pathways.

Think in real-life questions

The right route is easier to identify when we begin with what is actually happening in someone’s life rather than forcing needs into a service label too early.

“I need somewhere to live and ongoing support.”Supported living may be appropriate, depending on needs, compatibility and assessment.Explore options
“I already have somewhere to live but need support.”Community or outreach-style support may be more appropriate than a staffed supported-living scheme.Community support
“I’m preparing to leave hospital.”Transition, step-down and community reintegration may need to be planned alongside clinical and social care professionals.Moving to Moreways
“My needs or risks are complex.”More specialist support may be appropriate where mental health, behavioural, forensic or other complex needs overlap.Complex support
“I have a forensic history or section 37/41 restrictions.”Our forensic pathway explains how supported living can operate alongside statutory restrictions and supervision.Forensic support
“I want to become more independent.”Support can focus on skills, confidence, community participation and reducing unnecessary dependence.Greater independence
Support Can Change

The right amount of support is not always the same amount forever

Needs can increase, reduce or simply change shape. A good support model should be responsive rather than assuming that everybody follows the same pathway.

A flexible continuum

Support can move in either direction depending on wellbeing, risk, skills, circumstances and personal goals.

More StructuredHigher levels of planned support where needs or risks require it.
Supported LivingA home with individually commissioned support around daily life and outcomes.
Community SupportPlanned visiting or outreach support around particular areas of life.
Greater IndependenceMore tasks and decisions managed without paid support where appropriate.
What Matters To You?

Support should connect to the life you want to live

The purpose of support is not simply to fill hours. It should help create meaningful outcomes in the areas of life that matter to the individual.

Different People, Different Questions

Looking for support for yourself — or helping someone else?

The information you need may be different depending on your role, but the person receiving support remains at the centre.

If You’re Looking For Support

You may be thinking about where you want to live, what you want help with, what you would like to do more independently or what needs to feel different from your current situation.

  • You can ask questions without committing to a placement.
  • You can tell us what matters to you in ordinary language.
  • Your preferences, routines and goals matter alongside assessed needs.
  • Support should make sense in your everyday life, not only on paper.

If You’re Helping Someone

Families, advocates, carers and professionals often hold important information, but support should still be built around the person rather than around everybody else.

  • Share what has worked and what has not.
  • Help us understand communication, history and important relationships.
  • Raise worries early rather than waiting for crisis.
  • Where appropriate, stay involved in reviews and transition planning.
Different Starting Points

People can come to Moreways from very different places

There is no single route into supported living or community support. The transition needs to reflect the person’s current environment and what will be different next.

HospitalPlanned discharge and step-down with professional continuity.
Another PlacementMoving when current support no longer fits or a different model is needed.
Family HomeBuilding independence while maintaining important relationships.
Own TenancyAdding community-based support without necessarily changing home.
More Intensive SupportProgressing towards a less restrictive or lower-intensity model.

For Professionals

Looking to make or discuss a referral?

The Accessing Support pages explain the person’s journey. If you are a social worker, commissioner, clinician, brokerage professional or another practitioner looking to refer someone, our separate Referrals section contains the professional referral and assessment pathway.

Accessing Support

You can start with a conversation

You do not need to have every answer before getting in touch. Tell us what is happening now, what kind of change you are looking for and who is currently involved. We can help identify the most appropriate next step and direct professional referrals through the correct pathway.
