Care & Support Planning

Every person we support has a care and support plan built entirely around them. Developed together with the individual, their family and the professionals involved in their care, each plan sets out how support will be delivered day to day, the outcomes we are working towards and the steps that will help the person live as independently and safely as possible.









What It Means

Support that is planned around the person

A care and support plan is a living document. It describes what matters to the person, the support they need, how that support will be provided and who is responsible for delivering it. Rather than a fixed set of tasks, it is a shared agreement that grows and changes as the person progresses.

We develop every plan collaboratively, keeping the individual at the centre of every decision and ensuring families, commissioners and clinicians remain fully involved throughout.

The plan is designed to change as the person does. If needs increase, reduce or change in any way, we review and update the plan promptly and work with commissioners and professionals to make sure the right support remains in place.
