Assessment Services

Clear Evidence. Practical Decisions. Better-Matched Support.

Different Questions Need Different Evidence.

Select a route to see the question it answers and the practical output it can provide. Scope, timescale and professional input are agreed before work begins.

Provider Assessment

Can Moreways Offer A Suitable, Safe And Sustainable Arrangement?

Our standard referral assessment considers the person, required support, environment, compatibility, staffing, risks and transition requirements before a placement is offered.

Who Can Commission Us

A Defined Brief For A Real Decision.

An assessment begins by agreeing the decision to be supported, the evidence available, who must participate and the limits of Moreways’ role.

Current Focus

Placement requirements, functional needs, commissioning questions and recommendations for an achievable support model.

Two Clear Routes

Placement Assessment And Independent Work Are Not The Same.

Being transparent about the purpose of the work protects the person, the commissioner and the quality of the eventual decision.

01Part Of A Referral

Considering A Moreways Placement.

This provider assessment establishes whether Moreways can appropriately meet assessed needs. It informs suitability, service design, staffing, costing, compatibility and transition planning.

02Separately Commissioned

Answering A Standalone Assessment Brief.

Assessment-only work is commissioned against a defined question and produces a written report. Any relationship between the assessment and a possible Moreways service is declared from the outset.

The Assessment Visit

We Go To Where Life Is Happening.

Where appropriate, Moreways meets the person at home, in hospital or within their current placement. Scroll through the process to see how information becomes a usable recommendation.

Assessment Progress01 / 07
Step 01

Define The Decision.

Agree what the assessment must answer, who commissioned it, who will use it and what remains outside its authority.

Output: Confirmed Brief
01

Begin With A Precise Question.

A broad request becomes an agreed scope, evidence list, participation plan and intended output.

02

Make Involvement Possible.

Communication, timing, setting and trusted support are considered before the meeting takes place.

03

Read Before Asking Again.

Existing information is organised, tested for currency and used to identify important gaps.

04

See The Person Beyond The Paperwork.

The visit explores how daily life works, what already helps and where a different approach may be required.

05

Bring Relevant People Together.

Different contributions are considered without allowing the person’s own wishes and experience to disappear.

06

Check Whether The Model Can Work.

Recommendations must remain achievable within a real home, team, funding arrangement and network of responsibilities.

07

Finish With A Report People Can Use.

The final document separates evidence, interpretation, limitations and recommended next actions.

Inside The Report

Evidence Should Lead Somewhere Useful.

Choose a report layer. Each section has a distinct purpose, allowing decision-makers to see what was found, what remains uncertain and what should happen next.

Assessment Report01 — Findings

What We Found And How We Know.

The report identifies the source and relevance of the information used, differentiating records, direct observation, the person’s account and contributions from others.

Scope And Accountability

Clear About What The Work Can—and Cannot—Decide.

Moreways provides evidence and recommendations within the agreed brief. Statutory, funding, clinical and legal decisions remain with the body or appropriately qualified professional responsible for them.

Our Assessment Can Provide

A Provider Suitability Decision For A Moreways Service
Functional And Support-Needs Findings Within Our Competence
Placement, Compatibility And Environmental Analysis
Staffing, Transition And Care-Package Recommendations
Evidence For Wider Statutory Or NHS Processes

Our Report Does Not Replace

A Local Authority’s Statutory Decision Unless Formally Delegated
An NHS Continuing Healthcare Eligibility Decision
A Clinical Diagnosis Or Specialist Professional Assessment
A Court, Tribunal, MAPPA Or Mental Health Act Decision
Decision-Specific Capacity Work Outside The Assessor’s Role And Competence

Where occupational therapy, psychology, nursing or another regulated discipline is required, the scope and professional registration of the assessor will be confirmed before the work is accepted.

Assessment Questions

Clarify The Brief Before Work Begins.

Every assessment is scoped individually. These answers explain the broad service and do not confirm that a particular commission can be accepted.

01Is The Standard Moreways Pre-Admission Assessment Charged Separately?
Normally, no. Where a person is being considered for a Moreways placement, provider assessment forms part of our referral and suitability process. A separately commissioned report, specialist assessment or extensive work outside that process would be scoped and agreed in advance.
02Can An Assessment Be Commissioned Without Referring To Moreways?
Potentially, yes. Moreways can consider an assessment-only brief where the question falls within our competence, appropriate participation and information-sharing can be arranged, and any potential conflict of interest is identified.
03Will You Visit The Person?
An in-person visit will usually be recommended where direct engagement, observation or understanding the current environment is material to the question. The location, timing, communication adjustments and people attending are agreed beforehand.
04Can You Complete A Care Act Assessment?
A Care Act needs assessment is a local-authority function. An external organisation may undertake delegated assessment work only through an appropriate formal arrangement. Otherwise, Moreways can provide functional evidence and recommendations to inform the authority’s process, but does not replace its statutory decision.
05Can You Decide NHS Continuing Healthcare Eligibility?
No. Moreways can contribute relevant support evidence and participate in the process where requested, but NHS Continuing Healthcare eligibility is determined through the applicable NHS process and responsible integrated care board.
06What Information Is Usually Needed?
This depends on the brief. It may include current care and support plans, risk information, health or clinical reports, communication guidance, behavioural support plans, medication information, incident patterns, funding documents and details of the proposed environment.
07What Will The Commissioner Receive?
The agreed deliverable may include a written report, prioritised recommendations, proposed support or staffing model, transition dependencies and a feedback meeting. The format, intended audience and any implementation review are confirmed in the assessment brief.

A Better Starting Point

Begin With A Conversation, Not A Conclusion.

Good assessment creates enough space to listen, test assumptions and understand the decision in the context of the person’s real life.

Bring Us The Decision You Need To Make.

Tell us who the assessment concerns, the question it must answer, the available evidence and the required timescale. We will confirm whether Moreways is appropriately placed to undertake the work.
